Super Bowl XLVI game tickets face valued at
$900.00 each, Raffle Tickets Raffle tickets are $20 each. A maximum of 5000 raffle tickets will be sold. Tickets are available on a first come, first serve basis. There is no limit to the number of tickets an individual may purchase. Raffle tickets will be sold September 4, 2011 to January 7, 2012. Cash and checks will be accepted. Checks should be made payable to Saint Gregory Thaumaturgus Parish. Online raffle tickets will go on sale September 11, 2011 at www.sbraffle.org. Visa, MasterCard, American Express and Discover cards accepted. Individuals who buy raffle tickets online will be mailed their raffle ticket number. A raffle ticket stub will be completed and entered in to the Saint Gregory Super Bowl XLV Ticket Raffle. Online raffle ticket purchases will be charged an additional $1.50 processing fee associated with administrative costs. No raffle tickets or money for raffle tickets may be sent through public or private mail. A dishonored check will be considered an invalid raffle tickets and the entrant will be notified if this is the case. The Saint Gregory Super Bowl XLVI Ticket Raffle will be considered closed once the 5,000 maximum of tickets has been reached. In the case that the maximum number of tickets is not sold, the Super Bowl XLV Ticket Raffle will close on January 7, 2012. According to the Internal Revenue Service (IRS), raffles are games of chance that are not tax-deductible. Therefore, individuals who purchase Super Bowl XLV Ticket Raffle tickets may not deduct the cost of the ticket as a charitable donation. Selection of Winner The winning ticket will be drawn by an agent of an independent accounting firm on January 7, 2012 at 11:00 pm EST at Lakeview Country Club, North East, PA. The Auditing firm will use a random number generator to choose the winning ticket. Ticket-holders need not be present at the drawing to win. Winner will be notified within 24-hours of the drawing and will have one (1) week to collect the Super Bowl XLVI tickets from Saint Gregory Thaumaturgus Parish Rectory at 136 West Main Street, North East, PA 16428-1243. The winner will be notified by telephone, email, or U.S. mail, according to the contact information provided to Saint Gregory’s Church at the time of raffle ticket purchase. If the winner cannot be notified by January 13, 2012 after making reasonable attempts to do so, or if the winner is deemed to be ineligible, such winner will be deemed to have forfeited his or her prize and another winner will be selected for such prize. The name of the winner may be obtained by a request, in writing, to Saint Gregory Thaumaturgus Parish Rectory, 136 West Main Street, North East, PA 16428-1243 together with a self-addressed, stamped envelope, no later than January 31, 2012. Eligibility Individuals must be 18 or older to participate in the Super Bowl XLVI Ticket Raffle. General Terms and Conditions By purchasing a raffle ticket, you agree to be bound by these Official Rules and Regulations and the decisions of Saint Gregory’s Church, which will be final. Prize winnings are fully taxable under Federal and State of Pennsylvania law. The estimated value of the prize is $15,098. The winner will be issued IRS Form W-2G in this amount for tax year 2012. Saint Gregory’s Church will NOT be responsible for any individual income taxes over the amount included in the prize. The winner must complete form W-9 with a valid taxpayer identification number. The winner will be required to sign a publicity release permitting Saint Gregory’s Church to use his or her name, photograph, likeness and voice for any promotional purpose in any media. Saint Gregory’s Church is not responsible for any injuries, losses or damages of any kind arising in connection with or as a result of the winner’s acceptance or use of any prize. The National Football League, its member professional football clubs, NFL Ventures, Inc., NFL Ventures, L.P., NFL Properties, LLC, NFL Enterprises, LLC and each of their respective subsidiaries, affiliates, shareholders, officers, directors, agents, representative and employees (collectively, the ‘NFL Entities”) will have no liability or responsibility for any claim arising in connection with participation in this raffle or the prize awarded. By participating in this Raffle entrants agree that St. Gregory Church, its officers and directors and those involved in the development, organization, production, distribution including service providers, and any other person or entity associated with this Raffle shall not be liable for any and all claims, damages, losses or injuries, including any third party claims, arising from or relating to, in whole or in part, this Raffle. All entrants further agree that in the event of any third party claims, damages, losses or injuries, arising from or relating to a participant’s entry or participation in this Raffle, they will indemnify St. Gregory Church, its officers and directors and those involved in the development, production, distribution including service providers, and any other person or entity associated with this Raffle from any and all such claims, including attorneys fees related thereto. Any person attempting to defraud or in any way tamper with this Raffle will be ineligible for prizes and may be prosecuted to the full extent of the law. This Raffle shall be construed and governed by the federal laws and regulations of the United States, the state laws and regulations of the State of Pennsylvania, and local laws of the County of Erie, Pennsylvania. By participating in this Raffle you agree to submit to the jurisdiction of the Courts of the Commonwealth of Pennsylvania and those of the United States of America for any and all actions or claims related to this Raffle.
